The Department of Health has published an extended list of events that should never happen during care in the NHS.
Implementation of patient safety alerts - 'Too little too late'
This report shows that half of NHS trusts still have not complied with at least one alert which is past its deadline. 45 trusts had not complied with 5 or more alerts, some of which were years past the deadline.
